  • Nurse will schedule students appointment
  • Return completed consent form BY May 24th - student is not able to receive vaccine if not returned on time.

Letter To Families

Dear Parent(s) or Guardian(s) of a Child/Children in the School District of Arcadia:


Gundersen Tri-County Hospital and Clinics is offering a school-based COVID-19 clinic to vaccinate children within the school district who are at least 12 years old with the Pfizer vaccine.


The school-based clinic will be held on Thursday, May 27th, 2021 from 10AM to 12PM for first doses and June 17th, 2021 from 10 AM to 12 PM for second doses. Parents are welcome to attend with their child, but this is optional. It is required for parents to be available by phone during their child’s vaccine appointment.


Below you will find a consent form. If you wish for your child to receive the Pfizer COVID-19 vaccine, you will need to complete this form and return it to the school nurse by May 24st, 2021. Your child will not be vaccinated without your consent.

There is no cost for the vaccine. However, you will notice a section to add your insurance information. With your information and signature, you provide Gundersen Tri-County Hospital and Clinics permission to bill your insurance company for a small administration fee. This should not affect your insurance and will not be any cost to you.


The Pfizer COVID-19 vaccine is safe. The COVID-19 Pfizer BioNTech Vaccine EUA Fact Sheet for Recipients is available at: https://www.fda.gov/media/144414/download and must be reviewed prior to the clinic. There will also be copies at the clinic. Please contact Gundersen Tri-County at 715-538-4355 (ext. 28303) with questions or concerns after reviewing the EUA fact sheet.


If parent(s) or guardian(s) have not yet been vaccinated against COVID-19 and would like to be vaccinated with your child, please call Gundersen Tri-County at 715-538-4361 to set up your appointment to be vaccinated at the same time as your child.
